2018 NCMATYC Conference Registration
1) Refund Policy: No refunds will be issued after Feb 19th, 2018
2) Conference registration fees are the same for presenters and non-presenters.
3) Vendors should register with the Vendor Liaison, Jay Martin.
4) Receipts will be issued upon sign-in at the conference.
5) Registrants names & e-mail addresses will be provided to conference vendors upon request.
6) By attending the NCMATYC 2018 Conference, you agree to have your picture taken and used by the NCMATYC board, including, but not limited to publication in the NCMATYC newsletter.
This information will be used for the conference and for your NCMATYC membership.

Meal choices are requested to help with ordering food and keeping conference fees as low as possible. It will not be possible to change your meal choice at the conference.

Registration and Payment
Please select one of the following registration options. These fees will increase by $10 after 2/19/18.
$65 - Registration AND New/Renewed 3 year NCMATYC Individual Membership
$50 - Registration AND New/Renewed 1 year NCMATYC Individual Membership
$50 - Registration ONLY for Non Member, not Joining at this time or Lapsed Member not renewing at this time
$40 - Registration for Current NCMATYC Member
$10 - Graduate Student Registration
How will you pay your fees?
Online, using a debit card, credit card, or PayPal. (The links will be given after you fill out this form.)
Offline by sending payment to NCMATYC treasurer, Heather Barker. PO Box 73 Semora NC 27343
A copy of your responses will be emailed to the address you provided.
Never submit passwords through Google Forms.
