Incident Submission Form
Use this form to submit current and/or wave related incidents that occur on the Great Lakes to the National Weather Service. An incident is defined as current-related so long as a current was at least a partial cause of the incident. Most of the time, both waves and currents are the cause. An incident can be a rescue or fatality. You can combine multiple rescues/fatalities on one form if they occurred at the same time, but if they were at separate times, please submit a form for each incident. Thank you for helping us make our database more accurate! The goal of this database is to improve dangerous current forecasting, education, and outreach for the Great Lakes. Be as exact as possible. We prefer the beach name, county, state/province.

Any data you have will help. If victim/family doesn't want their name used, just include whatever information they are okay with us using. Their name won't be published anywhere. Only the gender, age, hometown of the victim is publicly available. Use "M" for missing. If you have to estimate age, do so-but let us know it is an estimate.
