Welcome to "The TALK" that Saves Lives: How to "Suicide Proof" Your Friends!
"The Talk" that Saves Lives ("TTSL") is a proactive, pure prevention program that helps prevent suicidal thoughts from getting or staying stuck inside someone's head.  

If you are under 18 years of age, a responsible adult must complete this form with you.

If you are 18 years of age or older, we assume you are responsible enough to fill this form out with yourself!

By filling out this form, and having "The Talk" you are helping to make teen suicide a thing of the past.

To participate in the TTSL program you must be at least 13 years of age. If you are between the ages of 13-18, fill in the TALKER section and have your RESPONSIBLE ADULT fill out the rest. If you are 18 years of age or older, fill out the entire form.
