Buddy League Canada Liability Waiver
I hereby give my consent for the above-mentioned athlete to play baseball under the auspices of BUDDY LEAGUE BASEBALL CANADA herein referred to as Buddy League and to abide by the rules of said program.

I hereby acknowledge that Buddy League provides limited insurance coverage and that I am responsible for any additional medical, dental or similar expenses not covered by the aforementioned policy that may be incurred as a result of any incident involving the above-mentioned athlete.

I agree that I shall be responsible for any personal injury or property damage caused by the above-noted athlete while participating in any activities of Buddy League.

The undersigned assumes all responsibility for any and all risk of damage or injury that may occur to the above-mentioned athlete as a participant in any programs by Buddy League. These activities may include practices, tournaments, skills sessions, games, transportation, and other activities related to the program. Further, the undersigned agrees to follow and abide by all Buddy League policies.

I understand that, in the event of a situation requiring medical treatment as deemed by a doctor, power of consent for the deemed treatment is granted to a Buddy League coach or Director in the absence of a parent/guardian. I understand that all Buddy League players are entitled to insurance coverage as facilitated by Buddy League.

In consideration of such the undersigned hereby releases and discharges all claims, demands, rights or causes of action present or future, whether known or anticipated and resulting from or arising out of or incident to the above-noted athlete participation in said program.

Photo/Video Release
Buddy League Canada may take photographs and/or video of my athlete to be used to promote the program. We require your consent to use these photos/videos of your athlete/dependent/self, and/or any copies of photos/videos in any editorial and/or promotional material produced and/or published by Buddy League Baseball Canada or other organizations in perpetuity. I understand that agreeing to this release does not guarantee publication of any media. I also understand that there will be no compensation or remuneration for the use of any media.

Concussion Code of Conduct (Rowan's Law)
I will help prevent concussions by:

• Wearing the proper equipment for my sport and wearing it correctly.

• Developing my skills and strength so that I can participate to the best of my ability.

• Respecting the rules of my sport or activity.

• My commitment to fair play and respect for all* (respecting other athletes, coaches, and officials).

I will care for my health and safety by taking concussions seriously, and I understand that:

• A concussion is a brain injury that can have both short and long-term effects.

• A blow to my head, face or neck, or a blow to the body that causes the brain to move around inside the skull may cause a concussion.

• I don’t need to lose consciousness to have had a concussion.

• I have a commitment to concussion recognition and reporting, including self-reporting of possible concussion and reporting to a designated person when and individual suspects that another individual may have sustained a concussion.* (Meaning: If I think I might have a concussion I should stop participating in further training, practice or competition immediately, or tell an adult if I think another athlete has a concussion).

• Continuing to participate in further training, practice or competition with a possible concussion increases my risk of more severe, longer lasting symptoms, and increases my risk of other injuries.

• I will not hide concussion symptoms. I will speak up for myself and others.

• I will not hide my symptoms. I will tell a coach, official, team trainer, parent or another adult I trust if I experience any symptoms of concussion.

• If someone else tells me about concussion symptoms, or I see signs they might have a concussion, I will tell a coach, official, parent or another adult I trust so they can help.

• I understand that if I have a suspected concussion, I will be removed from sport and that I will not be able to return to training, practice or competition until I undergo a medical assessment by a medical doctor or nurse practitioner and have been medically cleared to return to training, practice or competition.

• I have a commitment to sharing any pertinent information regarding incidents of removal from sport with the athlete’s school and any other sport organization with which the athlete has registered* (Meaning: If I am diagnosed with a concussion, I understand that letting all of my other coaches and teachers know about my injury will help them support me while I recover.)

• I will take the time I need to recover, because it is important for my health.

• I understand my commitment to supporting the return-to-sport process* (I will have to follow my sport organization’s Return-to-Sport Protocol).

• I understand I will have to be medically cleared by a medical doctor or nurse practitioner before returning to training, practice or competition.

• I will respect my coaches, parents, health-care professionals, and medical doctors and nurse practitioners, regarding my health and safety.

2025 Buddy League Canada Registration Fee
The registration fee for the 2025 Buddy League (Junior/Senior Division) baseball season is $70.00

This fee provides athletes with access to regular Monday and Thursday evening baseball field times at Kin 1 (Riverside Park), Buddy League & Blue Jays t-shirts, and access to equipment.

The registration fee for the 2025 Buddy League Learn-to-Play baseball season is $35.00

This fee provides athletes with access to regular Thursday evening baseball field times (5:30-6:30 p.m.) at Kin 1 (Riverside Park), Blue Jays t-shirt, and access to equipment, beginning in late June.

Any applicable tournament, transportation, or accommodation costs should be considered additional based on attendance at these events and not included the above noted registration fee.
