By checking the box I (Parent/Guardian) understand that participation in the basketball sessions involves physical activity and inherent risks, including but not limited to, the risk of injury. I acknowledge that serious accidents can occur in sports and that participants may sustain injuries, even with the best safety precautions. I release the coaches/facility from liability for injuries or damages resulting from my daughter's participation.
