Home Health Aide Hall of Fame
The Home Health Aide Hall of Fame honors Personal Care Service workers in
Direction Home Akron Canton’s four-county service area who have made
significant contributions to their agency, profession, and their clients
through their exemplary service and dedication.

Each year, one Home Health Aide from Portage, Stark, Summit and Wayne
County is chosen to receive the Hall of Fame award. We look to our
providers, Care Managers, and consumers to nominate dedicated workers
that represent a positive image of the home care profession.
Nominations for this award should be submitted no later than
October 20, 2023.

The winners and sponsoring organizations or individuals will be notified
once a selection has been made.

Guidelines for Nominating
Guidelines for Nominating
-- Nominee must be Home Health Aide employed by an agency contracted with DHAD and serving DHAD consumers in the Area Agency on Aging 10B four-county region (Portage, Stark, Summit, Wayne)
-- Major focus should be on the worker’s dedication to service, their agency, and the profession of home care. Factors to consider can include (but not be limited to): professionalism, dependability, compassion, trustworthiness, ethics, confidentiality

Nomination Materials Requested
-- Completed application form
-- Any supporting materials that would further support nomination
-- Sponsoring contact person name, agency (if affiliated), telephone number, and email address

The award recipients will be recognized in Direction Home Akron Canton publications
and promoted to local media. They will receive an award plaque and award recipients,
along with their family and friends, will be invited to the Hall of Fame hybrid event in
December to be recognized for their good works and to accept the award.

-- Have the services or achievements of the individual had an impact on the
employer and the consumers they serve?

agency and as a representative of the profession?
Unusual Circumstances / Difficulties
-- Has the nominee overcome difficulties or been a part of a unique field of endeavor
or improvement effort in some capacity?
