CHW Competency Exam
This is a form to sign up for a community health worker (CHW) Competency Exam. This is only for individuals who were registered on Oregon Health Authority (OHA) Traditional Health Worker Registry  and whose certification was not renewed within the 6-month grace period following expiration of their CHW Certificate. If you are uncertain if this applies to you please reach out to workforce@orchwa.org or the OHA Equity and Inclusion Division. 

Please allow for up to an hour for the exam. Once this form is completed you will receive an email confirming the date of your exam and detailed information about the test and recertification with OHA and payment of $150.
