Technology Rental Agreement Fall 2020-21
If your family does not have an adequate device in which your child can access the necessary learning materials on a daily basis, please review the Holy Hill Area School District equipment agreement and complete the form below.
 
I understand and acknowledge that the equipment provided to me by the Holy Hill Area School District is the property of the Holy Hill Area School District and is to be used in the course of district related tasks and not for personal use.
 
If the equipment is lost, damaged or stolen OR if damaged as a result of non-district related use and non-normal wear, I understand and acknowledge I am responsible to reimburse the district for any expenses necessary for repair and or replacement of equipment.
 
I understand use of computer and mobile device equipment is subject to all contingencies of the HHASD 7540.03 Acceptable Use Policy and any other directives from building or district administrators.

I understand that internet filtering is in use in our district network and while a student is logged in with his/her HHASD account. The district is not responsible for monitoring student activity on the device when the device is in use outside of the district network.  Inappropriate use of technology may result in the loss of device privileges and/or disciplinary consequences.
 
Students will:
·   Keep the device away from food and drink;
·   Keep the device in a well-protected environment away from extreme temperatures, pets, or on floors;
·   Report damage or device malfunction to the HHASD Technology Department immediately.
 
Students will not:
·   Modify the device in any way - this includes installing unapproved apps, extensions or software
·   Apply marks, stickers, or other decorations to the device, cover, or charge;
·   Allow others to use the device
 
Equipment shall be returned upon request and notification of Technology services for maintenance, updates, inventory and any other request.  All equipment must be returned to the student's school upon discontinuance of attendance at an HHASD school.
 
I give my student permission to take this device home from school. I agree that loss, theft, or damage to the device must be immediately reported to the HHASD Technology Department.  I acknowledge that I am responsible for the cost of repairs or replacement.
