In case of an accident or sudden illness, when a parent or guardian is unavailable, I authorize a school representative to obtain medical care for my student, including necessary transportation, in accordance with their best judgment. I further authorize the doctor named below to provide the care and treatment they consider necessary. If the physician designated below is unavailable, I authorize such care and treatment to be performed by any licensed physician or surgeon selected by the school representative. I agree to pay all costs incurred as a result of the foregoing. Please include Doctor's Name and Student's Medical Number.
Credit Recovery Summer Session Grading Policy : Students who participate in FCUSD Credit Recovery Summer Session will only be eligible to earn the highest grade of a C to recover A-G eligibility and to meet graduation requirements. Credit Recovery Summer Session Courses - Students take these classes to make up a failed course. *Students can ONLY take 2 courses*. (Five (5) units of high school credit will be given for each online course successfully completed. First semester courses are followed by an ‘A’; Second semester courses are followed by a ‘B’.)

Summer School Enrichment: Students must have an impacted schedule to take an enrichment course. An impacted schedule is defined as a full schedule of seven classes (no TA period) and one of the following criteria: A remaining need for classes that complete graduation requirements. A remaining need for classes that complete a-g requirements or IB courses. A remaining need for classes that complete a Career and Technical, Project Lead the Way, World Language, or Arts pathway. Summer School Enrichment courses must be approved by a school administrator or counselor.
