This form is used to lodge your expression of interest into an area of selected study with Amy Boleszny Pty Ltd RTO 30831 trading as Healthy Business Training Academy  (referred to as ‘the Academy'). Your information will be forwarded to a selected qualified vocational education trainer and assessor who will contact you to discuss your individual training needs. An enrolment form, training plan and cost will be provided based on this discussion.

Should an external body, such as another training organisation wish to verify your qualifications, they must give us your written permission for this information to be released.
