If you haven't already, please apply for a Private School Scholarship through Step Up for Students at
www.sinailearninglab.org/stepup. If requested when signing up please make sure to select
TEMPLE SINAI OF NORTH DADE (340) as your school. When you have received your Award ID number, please submit that here.
After submitting this form, we will enroll your child through Step Up in our program. You will then receive an email for you to authorize this enrollment. Each time we bill Step Up on your behalf you will need to authorize that billing by responding to the email they send you.
Not authorizing your child's enrollment or billing through Step Up for Students will result in funds not being sent to the school and full tuition remaining your responsibility.
