To complete this form you will need Drone Registration Number, TRUST Certificate, and depending on intent Remote Pilot Certification (Unmanned Aircraft General UAG exam). More information on these elements here:

Drone Registration is $5, valid for 3 years at FAADroneZone https://faadronezone.faa.gov 
Choose "I fly under The Exception for Recreational Flyer". Cost is a School Site or Department expense.

Teachers, Students, and Staff will need to take the TRUST exam at https://faatrust.thedroneu.com before you fly at Modest City Schools (free and 100% correctable) and upload certification to this MCS online form: https://mcsedtech.com/dronetrustcert 

If intent is to use drone for photography, videography, HVAC, or work use you will need Remote Pilot Certification and upload certification to this MCS online form: https://mcsedtech.com/dronepilotcert 
Remote Pilot Certification (60 questions, $175 for Knowledge Test). The cost is a School Site or Department expense. The test is in-person and maybe 30 miles or more from your current location with operating hours during the workday.
