Frederick County 4-H Club Purchase Request Form
  • This form is to completed for an individual 4-H Club purchase that is $500 or greater.
  • This request should be submitted by the club leader, club treasurer, or UME Volunteer responsible for the club financial records at least 1-2 weeks prior to the purchase.
  • A member of the County 4-H Team will contact with approval or questions within 1-5 business days of the request submission.
