For clients under the age of 18, a parent or guardian must accompany them or submit this form at the time or before the tanning session. All clients under 18 are required to wear a gender-appropriate swimsuit or disposable attire provided at the facility at all times.
Additionally, clients under 18 seeking topless tanning must have explicit consent from a parent or guardian. Please note that Hush Hush Tan reserves the right to refuse service if the individual filling out this form cannot be reached prior to appointment.
