Hotspot- Parent Request Form
Grandview C-4 School District has partnered with T-Mobile to support students in the free and reduced meal program. If you’re pre-qualified and do not have internet access at home, please complete the form below to request a district-provided device. Devices are limited and available on a first-come, first-served basis. They remain the ownership of the Grandview School District and should be returned when no longer needed are you leave the district.  Only one device is allowed per household.  Please review the guidance below:

  • Eligibility:

    • Students must be enrolled in the district and approved for Free or Reduced Lunch status.

    • Priority will be given to households with no existing internet access.

    • One hotspot will be issued per household, regardless of the number of students enrolled.

  • Usage Expectations:

    • Hotspots are intended for educational use only (homework, research, class projects, virtual learning, etc.).

    • The hotspot must be used regularly (a minimum of once every 14 days).

    • Internet activity will be monitored for compliance with district technology policies.

  • Non-Use or Misuse:

    • If a hotspot is not used for 30 consecutive days, it will be turned off and asked to be returned, as there is a limited quantity available.

    • Repeated misuse (streaming for entertainment, inappropriate sites, excessive non-educational activity) may result in device loss.

  • Accountability:

    • The parent/guardian must sign a Hotspot Agreement Form, acknowledging receipt, usage expectations, and the return policy.

    • Hotspots must be returned at the end of the school year or upon withdrawal from the district.

    • Families are responsible for returning the device in good condition with all accessories and cords.

  • Support:

    • Please get in touch with your child's school if there are issues with the hotspot.

    • Families who lose service due to non-use may reapply, but approval will be based on availability.

El Distrito Escolar Grandview C-4 se ha asociado con T-Mobile para apoyar a los estudiantes que participan en el programa de comidas gratuitas o a precio reducido. Si cumple con los requisitos y no tiene acceso a internet en casa, complete el formulario a continuación para solicitar un dispositivo proporcionado por el distrito. Los dispositivos son limitados y se asignarán por orden de llegada. Solo se permite un dispositivo por hogar.
