Parents/Guardians,

Please fill out this form concerning transportation. Please mark whether your child will be a walker, car rider, or bus rider. Also, if a car rider, indicate from where he or she will be picked up, and if a bus rider, include the bus number he or she will ride and the street address of the drop-off. This form will serve as your note and will be kept on file in our office. Please send a note on days when your child will have alternate means of transportation. Otherwise, he or she will be expected to follow this form.

Please note that WMS students will only be allowed to go to Warren High School parking lot if they are a car rider with a sibling who is a student there. Otherwise, WMS students will no longer be allowed to be picked up in that parking lot. All WMS students riding with siblings from Warren High School parking lot must have a note on file in our office. WMS students will only be allowed to go to Brunson if they are a car rider with a sibling who is a student there. Otherwise, WMS students will not be allowed to be picked up there. WMS students riding with siblings from Brunson Campus must have a note on file in our office. All WMS bus riders must be picked up at Warren Middle School campus. No WMS student will be allowed to  catch the bus at any other school campus. No exceptions. All students who are walkers must have a note on file in our office.
