Take Home Meals Registration
Parents,
When we are on Attendance Plans 1 or 2, students are eligible to order meals to go for the days that are not in school. If you would like to purchase these meals, please indicate below each week you would like your child to receive take home meals. You must complete one form per child, per week. If you choose to order, students will receive 4 breakfasts and 4 lunches. The meals will be frozen and available for pick up at the end of the day, at which time students will pay using their ID to scan for their lunch account, as usual. The cost of the meals will be based on your child's current lunch status (Free, Reduced, Full-Pay). On Attendance Plan 1, full pay students will be charged $10; students on Reduced status will be charged $1.60; students on Free status will receive all meals for free, per USDA guidelines.

Sample of Items that will be sent home for students:
1 frozen pancake
1 frozen cinnamon roll
1 frozen Belgian waffle
1 frozen banana bread

1 frozen pasta bowl
1 frozen three bean chili bowl
1 frozen Alfredo bowl
1 frozen Grand strand sandwich
1 frozen roll
4 apples (= full cup serving)
4 apple sauce cups

Additional items that students may take:
1 celery
1 baby carrots
1 garden salad
1 bean salad
1 - ½ gallon milk

The form below will take us through December 2020. A new form will be available in January 2021, if necessary. You may complete additional forms to order ahead through December.

ORDERS MUST BE PLACED BY 5PM ON THE FRIDAY BEFORE THE WEEK OF PICK UP.
