    Every two-bit amateur phisher can create a form just like this, only a little more believable. Even if they only get one victim per thousand messages they send out, it's still worth the effort and is a good criminal way to make money. Your valid ID and password can probably be used for other things in addition to access to your email account. It might be useful to login to Banner and change your Direct Deposit for your paycheck or your financial aid deposits. Be an Internet Skeptic, not a Phishing Victim!
