Unsportsmanlike conduct by coaches and/or players and/or spectators players and will not be tolerated. The following essential elements of the Code of Conduct must be followed: 1. Sportsmanship and teaching the concepts of fair play are essential to the game and must be taught at all levels and developed both at home and on the field during practices and games. 2. The value of good sportsmanship, the concepts of fair play, and the skills of the game should always be placed above winning. 3. The safety and welfare of the players are of primary importance. 4. Players should always demonstrate positive behavior and respect toward teammates, opponents, coaches, officials, parents and spectators. 5. Coaches, players, parents and spectators are expected to demonstrate the utmost respect for officials and reinforce that respect to players/teammates. 6. Grievances or misunderstandings between coaches, officials or any other parties involved with the organization should be communicated through the proper channels and procedures, never on or about the field of play in view of spectators or participants. 7. Spectators involved with the game must never permit anyone to openly or maliciously criticize, badger, harass or threaten an official, coach, player or opponent. 8. Eligibility requirements, at all levels of the game, must be followed. Rules and requirements such as age, previous level of participation, team transfers, etc, have been established to encourage and maximize participation, fair play and to promote safety. *

A signed waiver is mandatory prior to practice, participation or game. Please read the entire waiver before signing. In consideration of being allowed to participate in any program with the Lake Effect Lacrosse program, related events and activities, the undersigned acknowledges and agrees that: 1. The risk of injury from activities involved in this program is significant, including the potential for permanent injury, paralysis and death and while particular rules, equipment and personal discipline may reduce this risk, the risk of serious injury does exist; and 2. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASES or others and assume full responsibility for my participation; and 3. I willing agree to comply with stated and customary terms and conditions for participation. If however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official and/or coach immediately; and 4. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS, Lake Effect Lacrosse, their officers, organizers participants, their agents, coaches, employees, owners, lessors of premises used to conduct the event (RELEASEE) WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASES OR OTHERWISE. I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT AND ACKNOWLEDGE THAT I FULLY UNDERSTAND ITS TERMS AND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT AND THAT I SIGNED THIS FREELY AND VOLUNTARILY AND WITHOUT ANY INDUCEMENT. *
