Group Membership Request
This form is for schools, districts, or organizations who are requesting to join as a COSEM Group Member. This means that you are providing a Professional Membership to 6 or more members from your school or organization for one year.

Steps: 
1. Complete this form. 
2. You will receive an email from hello@educatingmindfully.org within the next few days. In the email will be an invoice, a credit card payment link if applicable, and instructions to join our Member Site for your Group.
3.We will reach out to your Main Contact in a year to find out if you’d like to renew. We hope you find COSEM to be valuable!
