I understand that Madison Athletic Department procedures require that students ride the bus to and from all athletic events and a departure from this requirement will release the Madison Board of Education/Madison Athletics from all liability for any adverse results that may occur.
NOTE: All students participating in away games and another official, school-sponsored, group events for which transportation is provided, are expected to travel to the event and return to school together, on school transportation. All participants are to remain under the supervision of the coach(es), or other school officials or designated chaperone(s) at all times, until they return to school. When school transportation is provided, participating students MUST ride it both ways. Exceptions to this rule may be allowed only with advance approval from the athletic director, or other administrators who will grant exceptions only for emergencies, hardships, or other unusual, extenuating circumstances.
I agree to release the Madison Board of Education and its employees from all liability with reference to the above-stated transportation rule. I also understand that if these rules are not followed and I drive my child without approval, or he/she drives themselves, to an athletic contest, he/she will not be allowed to compete.
***Student athletes, regardless of age, will not be allowed to drive to or from contests. Please make sure to identify the adult delivering or picking up your student athlete so it can be verified by the coach/school designee. ***
Please not that student athletes will be released to their parent/guardian ONLY. Student athletes will not be released to another students parent/guardian without prior approval from a school administrator.
