Oxford United in the Community are asking as part of obtaining an Activity pack, we need to collect some information as the packs are produced as all part of the on going funded Premier league primary stars programme.
Oxford United in the Community (OUitC) and Premier league primary stars wish to obtain your explicit consent to process information about you for the following purpose.

- In order to evidence the level of participation on the programme and to continue to draw down and continue future programme funding, by exploring whether the programme is achieving its intended outcomes.

All the information we hold about you will be shared anonymously and aggregated (not individual data) with the Premier League for research and evaluation purposes only.
