Please submit this form once per entry. For couples, either leaders or followers can submit the video entry for the couple.

1. Please confirm your participant and entry information.
2. Provide a Youtube link for your video entry. Make sure the Youtube video permissions are set to "unlisted" or "public" so the judges can view your dancing.
3. Confirm that you agree to the terms detailed in the Participation Waiver.
