ONLY TOWN PROGRAM LEADERS SHOULD BE FILLING OUT THIS FORM

Use This Form to Submit a Waiver for approval to have a child who does NOT live in your town programs geographic assigned area to be allowed to play for your town program for CLASSIC PLAY. 

If the player you are filling out this form for lives in a town that has an MYL program, before you submit this form, you MUST contact the town program in which the player resides to ask their recommendation on if the waiver should be approved. 

When we have the updated 2026 town leader + boys/girls director contact list, it will be linked here.
