Please fill out this form for each youth member attending a mission trip this summer. This reservation form will secure their spot on the trip. Further forms will be filled out at the Mission Trip Prep Meeting.

Each participating youth, along with one parent or guardian, must attend the Mission Trip Prep Meeting on Sunday, May 15th at 4pm at Promise Road. If you are absolutely unavailable to attend, please contact Sam as soon as possible.

A $200 deposit for each trip will be due on May 15th. This is also the last day to reserve and register spots for the trips.

High School Trip #1: Fort Worth, TX ($600)
June 19th-25th

High School Trip #2: Iowa: Christian Conference Center ($450)
July 3rd-9th

Middle School Trip: Nashville, TN ($400)
July 17th-23rd

Grade Clarifications
High School: 8th-12th graders
Middle School: 6th-7th graders
(Based on the grade completed at the end of the 2021-2022 school year)
