This form is to make submitting your Biography and Art Information easier, in lieu of emailing the word document back.  Some duplication of information may be asked of you. Please make sure you read the 2022 Fresh Impressions Artist Information Packet before submitting your information.

The Fresh Impressions 2022 Entry Form & Consignment Agreement paper copy must be submitted with your Art to your Art Instructor.
