Puppy Application
If you are interested in one of our puppies, or getting on the waitlist for a future litter, please complete the form below.
This application is required to be considered for a Mill Dam Creek puppy. It helps us get to know you a little bit, and helps us determine if this breed, or a specific puppy will work for you.

Filling out the application is NOT a commitment to buy, NOR is it a guarantee you will be approved for a puppy.

We will contact those on the waitlist when breeding has taken place, and again when pregnancy is confirmed. We will only take 1-2 deposits upon pregnancy confirmation. After that, we will not take deposits until puppies are 2 weeks of age.

If you have found a puppy elsewhere and are currently on our waitlist, please let us know!
We will not be offended in any way, we would just like the courtesy of knowing so we can continue to take applications for the litter.

Price of our puppies for 2020 is $1,450 regardless of coat or eye colour. A $250 non-refundable deposit is included in this price. *This price reflects pet only/non-breeding limited registration*

Merle seems to be an extremely popular coat pattern. (We love them all and have no preference...a good dog comes in any colour!) Please note that if your preference is for a Merle coated dog, you may have to wait for a future litter depending on the number of people on the waitlist ahead of you, as we have no control over what our girls give us :)
Do you have a colour preference? *
You may select more than one
Required
Do you have a gender preference? *
We like to wait to make final placements until minimum 6 weeks of age to get an idea on personalities to ensure the best fit for your home. Do you have a problem with this? If yes, please explain why. *
Please note: if we are keeping a puppy back in our program, puppy selection may not occur until 7-8 weeks of age, as we need to wait to best evaluate structure on potential breed/show prospects
