2017 CJMEA Concert Band Festival Application
Please complete the items below to submit your request to perform. Performance dates will be given on first come, first served basis AND based on scheduling needs (e.g., trips, musicals, etc.) Please be as flexible as possible. We will limit each night to a maximum of nine ensembles.

The first ensemble from a school requires a $200 participation fee.
Each additional ensemble requires a $150 participation fee.
Once the form is completed, an invoice will be sent to reflect the appropriate fees for your school. Please allow up to 1 week for the invoice to arrive.
