This form is used for students with disabilities who are in need of housing accommodations at Malone University. Please note that appropriate documentation must be provided to Student Accessibility Services in order to be approved for a housing accommodation. *Emotional support animals and service animals are permitted in every residence hall, however, there are floors that are designated animal free. This is to accommodate other residents who have allergies and/or are not comfortable living in community with animals.

**Please note that there is an additional charge of $625 per semester for a single room. This fee is not waived for medical reasons. Requesting a single room/housing accommodations for medical reasons gives students with disabilities priority housing; it does not change the cost of the room.
