The player listed will participate in tryouts for Kingdom KC Volleyball Club. I certify that to the best of my knowledge the participant named herein is physically fit to engage in the above activities. I acknowledge and agree to the following terms and conditions in consideration of the player listed being allowed to participate in tryouts organized by Kingdom KC Volleyball Club.

**Assumption of Risk**: I understand and acknowledge that participating in tryouts involves certain risks and hazards, including but not limited to, the risk of personal injury and property damage. I voluntarily assume all risks associated with my participation.
**Release of Liability**: I hereby release, waive, discharge, and covenant not to sue Kingdom KC Volleyball Club, its officers, directors, employees, agents, and representatives from any and all liability, claims, demands, actions, or causes of action whatsoever arising out of or related to any loss, damage, or injury that may be sustained by me while participating in tryouts.
**Indemnification**: I agree to indemnify and hold harmless Kingdom KC Volleyball Club, its officers, directors, employees, agents, and representatives from any and all liabilities or claims made as a result of my participation in tryouts, including but not limited to, attorney fees and costs incurred in defending such claims.
**Medical Treatment Authorization**: In the event of an injury or medical emergency, I authorize Kingdom KC Volleyball Club and its representatives to obtain medical treatment for the player listed as deemed necessary. I understand that I am responsible for any medical expenses incurred as a result of such treatment. I understand that the staff of Kingdom KC Volleyball Club cannot administer any medications, prescriptions or non-prescription, to participants.
**Compliance with Rules and Regulations**: I agree to abide by all rules, regulations, and instructions provided by Kingdom KC Volleyball Club and its representatives during the course of tryouts.

I have read this waiver and fully understand its contents. I acknowledge that I am signing this document voluntarily and of my own free will.
