I certify that my dog is in good health and has received all required vaccinations, including but not limited to rabies, distemper, and Bordetella. I understand that my dog must be free from any contagious diseases or parasites to be eligible for boarding. *

In the event of an emergency or if my dog becomes ill during their stay, I authorize Doodles and Deadlifts LLC to seek immediate veterinary care from a licensed veterinarian. I agree to be responsible for any medical expenses incurred during my dog's stay.
