IMPORTANT: In order to be considered for co-sponsorship, a proposal must be submitted at least 6 weeks prior to the date of the event. This is so that materials can be reviewed, edited, and submitted to CBMT for approval in time to meet required CBMT deadlines.
Please complete and submit the following online application for co-sponsorship of a CMTE event with SWAMTA. You will be responsible for the 75.00 CBMT fee, following promotional materials regulations, collecting attendance and evaluations, and giving participants certificates of attendance. In an effort to maintain quality of service and decrease continuing education costs for the region the SWAMTA board has instituted a 5% of profits fee for use of the SWAMTA approved provider number and co-hosting efforts. The continuing education chair will supply you with a payment form to complete and send to Amy Rogers when collecting your post course evaluations and attendance.
In addition to this application you MUST email a current curriculum vitae (CV) or resume for EACH presenter to SWAMTA Continuing Education Director Jennifer Townsend, MMT, MT-BC at email@example.com, as well as all promotional materials for approval.

Proposals are reviewed through a blind process. Information for the blind review will be taken from your submission form. Please do not include any identifying data in the presentation overview. All areas of the form must be completed to be considered for a co-sponsorship in addition to the CV/Resumes for each presenter being emailed to firstname.lastname@example.org. Proposals will be rated on the relevance to CBMT Board Certification Domains, clarity of writing and completeness, suitability of location, and widespread appeal.
