This form needs to be completed by competitors in Forest Spring events to provide entry information and evidence of communication and agreement with additional precautions. It should be sent no later than the Monday evening before the event.  Should someone develop Covid-19 symptoms after submission of the form, please do not attend the event.

Charges for each Forest Sprint event will be £5 for adults and £2.50 for Juniors/Students. A payment invoice for all Forest Sprint events that you enter will be sent to you at the end of the series and your email address will be kept until that time. By submitting this form you are agreeing to that, and to pay when requested.
 
Accompanying persons are people who attend the event and participate in part of it but are not competitors (e.g. helpers, people shadowing competitors, event officials)
