All other Sponsors/Exhibitors who would like to attend conference sessions and receive Continuing Professional Education (CPE) credits must register separately for the Annual Meeting. Please visit http://www.eatwellmd.org/page/annual-meeting for conference registration information.
Payment Information
All sponsors must meet guidelines established in MAND’s sponsorship policy http://www.eatwellmd.org/page/sponsorship-policy and be approved by MAND’s Sponsorship Committee. Once approved, an invoice will be sent to the email address listed above with instructions for making a credit card or check payment.

The deadline to register is midnight EST Friday March 1, 2019. Registration will not be considered complete until payment has been received. Late registration may be accepted by email, based on availability. However, for all registration forms received after the March 1 deadline, a late fee of $50.00 will be added to the total cost.

Cancellation policy: Prior to midnight EST on Friday March 15, 2019, organizations that have completed registration are eligible for a 50% partial refund in the event that they are no longer able to attend the Annual Meeting. Unfortunately, we are unable to provide a refund for cancellations after March 15, 2019.
