What: This training is intended to increase the understanding of tribal providers of the DDS Regional Center Vendorization process. 

There are a total of two parts to this process that you will be asked to complete after registering for this training. 

• Pre-Survey: The pre-survey measures your knowledge prior to training session. This survey should take approximately 15-20 minutes to complete. 

• Post-Survey: The post-survey measures your knowledge and suggestions after you have completed the training session. This survey should take approximately 15-20 minutes to complete. 

Evaluation: To effectively evaluate this project the CTFC is completing pre and post testing of all participants. Please fill out the following survey questions to the best of your knowledge and ability. The results of the pre/post testing for this evaluation will be collected and analyzed by lead Evaluator Dr. Cutcha Risling Baldy. Data will be made anonymous and no identifying information will be included. However, we need to collect your identifying information on each pre/post test so that we can align the results before and after the training. Once those results are aligned, data will be cleaned so that identifying information will not be associated with data analysis, results, or published conclusions. For qualitative questions we encourage you to try to not provide details that may allow someone to identify you or those you may share information about. We will also make every effort to make anonymous any qualitative response that is provided to us before publication. If you have

There are only four required questions for the surveys: your name, email, and job title, and program manager. Please note that these surveys are not a test and will not affect your participation in the training. We hope that you will answer to the best of your ability without further research or attempting to look up the information before answering. 

Dissemination: Data collected from these surveys will be utilized for internal analysis and revision of future training and training curriculum documents. CTFC may also use the data in future reports that may or may not be published or available to the public. All data utilized in these reports will be anonymous.
