Talawanda College Credit Plus (CCP) Parent/Guardian Acknowledgement Student Intent Form
Please complete this form if you attended the in person presentation and/or once you have watched the CCP Information Presentation available HERE. 

By signing this form, you are acknowledging your understanding of the risks and benefits associated with taking CCP classes during the 2025-2026 School Year.

Please submit your INTENT FORM, this Google Form, and your college CCP application by April 1st.
There are no exceptions to this deadline. Please complete a separate form for each student.

We understand if an entrance exam score is required it is our responsibility to complete this by application deadlines set by each individual college.  *

School Counselors are available to answer questions but students are responsible for meeting all deadlines.

We understand the possibility of exposure to mature content exists for our student when taking college level courses.   *
CCP courses mark the beginning of your student's college transcript. By starting a college GPA early, it's important to note that any grades below a 2.0 in these courses can impact eligibility for financial aid. Additionally, the grades earned in CCP courses can influence college admissions decisions. *
Policy states that students may only take 30 credit hours under College Credit Plus. This includes both college courses and any high school courses. Students must turn in a detailed schedule of college courses to their school counselor by the start date of each semester for tracking and attendance purposes. Students who register for more that 30 credit hours will be responsible to pay for the entire course that placed them over the 30 hour limit. Students will be charged at the full college tuition rate. *

Hours Calculation:

1) Determine the student’s number of high school ONLY units.

2) Multiply that number by 3.

3) Subtract the result from the number 30.

4) That number is the total number of college credits that CCP students may earn that academic year.

Successful completion of a three or more credit-hour college course will result in 1.0 Carnegie unit earned at the high school.

Responsibility for transportation rests with the student. *
If you are a student-athlete, you must remain eligible in accordance with the OHSAA bylaws. To be athletically eligible, students must pass five, one-credit courses or the equivalent per grading period with the high school and college courses combined. Students must submit their midterm grades to the athletic office to remain eligible. *
Secondary schools are responsible for providing instructional tools ("books") for their CCP students. Students must pick up and return their textbooks independently from the university bookstore.
Textbooks MUST be returned by the end of the college semester.

TSD will seek reimbursement from students/families under the following two circumstances:
1) If the student receives a failing grade at the end of the college course; or
2) If the student withdraws from or drops the college course subsequent to the 14th calendar day after the particular course began; unless the student is identified as being economically disadvantaged.

Under Federal and State law, participating male students are required to register for and provide their Selective Service number to the public college within 30 days of their 18th birthday. *
Students are not allowed to leave early or arrive late to their THS classes due to CCP classes. *
I/We understand the student is the one who has access to the grades and will need to allow the parent/guardian to have access to the grades. *
By signing this form, I/We confirm that we either attended the CCP Parent Information Night on February 20th or reviewed the recorded presentation available on the THS Counseling page. *
The submission of this form serves as notification of my student's intent to participate in the CCP Program for the upcoming 2025-26 academic year.  *
