I fully understand and acknowledge the risks and dangers that exist in my use of all amenities and equipment in the Ballyboughal GFC Gym, and, that my participation in any activity, class, program, personal training or instruction carry the same risks and dangers.

My participation and use of gym equipment in such activity may result in my injury or illness, including but not limited to bodily injury, fractures, partial and/or total paralysis, eye injury, blindness, dismemberment or other ailments that could cause serious disability or death

By my participation in these activities and/or use of equipment, I hereby assume all risks and dangers and all responsibility for any losses and/or damages, whether caused in whole or in part by the negligence or other conduct of the agents, Officers, employees of Ballyboughal GFC club or other third parties;

I fully understand and acknowledge that the use of gym equipment may involve strenuous physical activity including, but not limited to, muscle strength and endurance training, cardiovascular conditioning and training, and other various fitness activities.

I acknowledge that I am physically fit and mentally capable of performing the physical activity I choose to participate in and do not suffer from any known disability or condition which would prevent or limit my use of the Ballyboughal GFC gym facility and its equipment.

I hereby acknowledge my responsibility in communicating any physical and psychological concerns that might conflict with participation in activity.

I fully understand that I may injure myself as a result of my use of gym equipment and I hereby release Ballyboughal GFC and its agents from any liability now or in the future for conditions that I may obtain. These conditions may include, but are not limited to, heart attacks, muscle strains, muscle pulls, muscle tears, broken bones, shin splints, heat prostration, injuries to knees, injuries to back, injuries to foot, or any other illness or soreness that I may incur, including death.

After having read and understood this waiver and knowing these facts, I ,on behalf of myself, my personal representative and my heirs, hereby agree to release, waive, discharge, hold harmless, defend and indemnify Ballyboughal GFC club and its agents, Officers and employees from any and all claims, actions or losses for bodily injury, property damage, wrongful death or otherwise which may arise out of my involvement and participation in the gym facility
