If you are using this form as a Club Member, your Primary Club is the club you joined first this year, and where you paid your OUC membership fees.
Club Executive *
Independent Members should select the blank option at the top of the list. 
Club Members may indicate their position on their club's Board of Directors, if any. Otherwise, select the [ BLANK ] option at the top of the list. 
OUC Membership Level *
Blue Member is the standard membership level at $50.
Gold Members pay a higher fee of $67 for voting privileges at the AGM. 
Gold Lifetime Members: This category is reserved for recipients of the Lt. Cdr. Ken Grant Memorial Trophy who do not pay an annual fee.
Minors DOB (Date of Birth)
Minors are under 18 years of age in Ontario. Check the age of majority in your province.
