Thank you for your interest in participating with the Canada Excellence Research Chair (CERC), Professor Karen Soldatić (School of Disability Studies, Toronto Metropolitan University (TMU)), in the program focusing on Health Equity and Community Wellbeing (HECW).

This form serves as the initial application to participate in research with the CERC HECW Program at TMU.
