Discipleship Tutorial Course Application Form
Please submit this form for EVERY student who will be attending the Discipleship Tutorial in 2018-19. This form is a contract to register, after completing the form students may add classes but CANNOT drop classes. We depend upon this commitment to be able to make sure we meet tutor minimums for each class, so if you are unsure please only register for classes you are willing to complete commit to at this time.

Every time block must be completed. You can indicate if you will not be at DT for a time period, otherwise your student must be registered for a study hall (study hall is $50 for the year, $75 if taking a Lego Robotics/Programming study hall).

Classes that do not meet the minimum requirement will be dropped, and students will be given the option to sign up for another class, study hall, or to stay off campus.

A non-refundable deposit of $100 (going towards administration/registration fees) is due upon completion of this form, and can be turned in at Discipleship Tutorial on Tuesdays or mailed to:

REGISTRATION DATE IS APRIL 26 FROM 9:00 A.M. - 1:00 P.M. A second registration date will be offered in July, but there will be an additional $25 fee per student if checks are not received by April 26th.
