Peak Performance Pre-Registration Form
Please complete the form below regarding Peak Performance Clinics. It will be helpful to know approximately how many students will be participating so we are able to provide adequate space accommodations. The cost of the clinic is $35 (which includes a clinic t-shirt). Optional meals will be provided for an additional $5 (covers lunch), and you can pay for that at registration. In addition, we recommend that you bring water (in a closed container) and a light snack. If supplies are available, we will have additional Paramount souvenirs for sale at the event.
