4. PLEASE TYPE YOUR EMAIL ADDRESSES CORRECTLY.

5. Your schedule may have a different name for your music class than the options on the form. Please select "Orchestra" if
your schedule says Chamber Orch, Intermediate Orch, Orchestra, or Advanced Orch. Please select "Band" if your
schedule says Symphonic Band or Wind Ensemble.

6. If a required field does not apply to you please enter N/A.
