With the current rate of COVID-19 in the general community and that a large percentage of the population is voluntarily locking down, the club committee would appreciate some feedback from our members. Can you please answer these questions.

Vaccination Status *

I have not been vaccinated

I have received one vaccination

I have received two vaccinations

I have received a booster shot

I have contracted COVID-19 and isolated for 7 days

We need at least 25 members to attend to cover the rent for Cavanbah Hall. How likely are you to attend meetings? *

I will definitely be there on 7th February

I will be there on the 7th if there are no more Covid outbreaks

I would rather wait a couple of weeks until Covid cases fall

I won't attend meetings until the virus is gone!

Although NSW Health no longer mandate vaccination certificates, what are your views? *

I don't have an issue mixing with unvaccinated people

I would prefer if all members were vaccinated
