Policies & Procedures
• You must submit this completed application AND a non-refundable deposit ($200) before your application will be processed and reviewed. If for some reason you are not invited to be a part of the team, your deposit will be refunded.

• Participants must adhere to rules outlined in the team covenant. Please read before applying.

• No one will be considered or accepted as a team member until a completed application is received.

• Short-term “GO” trips can be rewarding and life-changing; however, they can also be stressful. Please consider factors in your personal life at this time that may distract and prohibit you from fully committing to the mission of the trip and adapting to unusual conditions.

• Once accepted, team members are expected to attend all team meetings.

• If you are unable to participate in your trip, the 'go team' leader must receive cancellation notice as soon as possible. You will be responsible for all trip costs already expended on your behalf. Monies put towards “GO” trips are contributions, and the Internal Revenue Service prohibits the refund of contributions.

• If you have physical limitations, please apply for a trip in which you are physically able to participate. Some trips may be prohibitive for certain physical conditions.

• Team members will be given information regarding passports, visas, and vaccination recommendations from the Department of Health. Passport, visa, and vaccination costs are not included in the trip costs and are the responsibility of the team member. Team members assume the responsibility and liability for their personal health decisions.

• Additional information regarding the price and dates for each mission trip is available from the Go Team leader. Team meetings will review in depth information to prepare you for the mission.
My Commitment
As a member of this team, I agree to:

1. Remember that this experience is not about me. It's about what Christ wants to do through me, in me and for others. That's a big difference.

2. Remember that I am representing Turning Point Church and, more importantly, Jesus Christ. I will model Jesus in my behavior and attitude.

3. Remember that I am a guest working at the invitation of my hosts. I will remember the missionary’s prayer, “Where you lead me I will follow; what they feed me I will swallow.”

4. Remember that we have come to learn, as well as to teach, I will resist the temptation to inform our hosts about “how we do things”. I will be open to learning about other people’s methods and ideas.

5. Respect the host’s view of Christianity recognizing that Christianity has many faces throughout the world and that the purpose of this trip is to experience faith lived out in a new setting.

6. Develop and maintain a servant attitude toward all nationals and my teammates.

7. Respect my team leader(s) and his/her decisions.

8. Refrain from gossip.

9. Refrain from complaining. I know that travel can present numerous unexpected and undesired circumstances, but the rewards of conquering such circumstances are innumerable. Instead of whining and complaining, I will be supportive.

10. Attend all team meetings before the trip as well as any follow-up meetings.

11. Remember not to be exclusive in my relationships. If my boyfriend/girlfriend or spouse is on the team, we will make every effort to interact with all the members of the team. If I am attracted to a teammate, I will not attempt to pursue a relationship until after we return home.

12. Refrain from any activity that could be construed as romantic interest in a national or teammate.

13. I am not involved with illegal drugs, and I am aware that consumption of alcoholic beverages or the use of tobacco while on this trip is prohibited.

14. Refrain from the teaching or practice of any belief that would not be endorsed by Turning Point Church.

15. I agree to abide by the fund procedures endorsed by Turning Point Church and will personally thank all financial donors that contribute for my trip.

16. Remember that I can be sent home if I do not adhere to this Covenant or if my Team Leader(s) believes it is in my best interest or that of the team.
Assumption of Risk & Insurance Election Short-Term “GO” Trips
In consideration of my acceptance as a short-term volunteer with Turning Point Church, Bonita Springs, Florida, USA, I represent and agree that:

1. I am aware of the hazards and risks to my person and property associated with serving in a missions capacity, such hazards and risks including, but not being limited to, death or injury by accident, disease, war, terrorists acts, weather, conditions, inadequate medical services and supplies, criminal activity, and random acts of violence. I accept my assignment with full awareness of these risks, and subject to the insurance coverage described below, I voluntarily assume all risks of death, injury, illness, and damage to myself or any member of my family associated with such risks, and any damage to my personal property. I further recognize that such risks have always been associated with missionary service. 2 Corinthians 11:23-28

2. I attest and certify that I have no medical conditions that would prevent me from performing my duties.

3. Subject to insurance coverage provided, I waive and release any and all claims for damages which I, or my heirs or successors, may have against Turning Point Church or any agent or employee of any of such organizations, arising from my death, injury, or illness, or any property damage or loss occurring during the term of my assignment or as a result of my assignment.

4. In the event that I have minor children who will accompany me on my assignment, I, acting both on my own behalf and in their behalf as their parent and legal guardian, and subject to the insurance coverage described below, do hereby assume all risks of death, illness, or injury that they may suffer as a result of said assignment from those causes described above.

5. I expressly waive any defense to the enforcement of any provision of this commitment arising from a claim of lack of consideration and warrant that this commitment constitutes a legal, valid, and binding obligation upon me enforceable against me in accordance with its terms.

I expressly agree that this assumption of risk and indemnity agreement is intended to be as broad and inclusive as permitted by law. I further state that I HAVE CAREFULLY READ THE FOREGOING ASSUMPTION OF RISK AND UNDERSTAND ITS CONTENTS; AND I VOLUNTARILY SIGN THIS RELEASE AS MY OWN FREE ACT.
