IDA S. BAKER SIGN OUT FORM
Please use this form to request the release of your IBHS student. Once submitted, this form will be automatically sent to the attendance department. Please allow at least thirty (30) minutes for the release of the student from the time of submission.

This form is for student drivers, walkers and bike riders ONLY.

District Policy states students will not be released within the last 30 minutes of the regular dismissal time.

1. It is recommended that electronic sign outs be submitted prior to 10am, even for a later dismissal time.This will give our staff time to call and verify the sign out. No sign outs after 1:30pm per District Policy.

2. The Electronic Sign Out form must be used. Email requests will not be reviewed. Over the phone sign outs will be used for emergencies only.

Parent/Guardian will be contacted by the school to confirm the electronic sign out request. The phone number given must match the parent/guardian information currently on file in FOCUS. If it does not, your child must be signed out in person, with proper identification.
