Eligibility Criteria: To be eligible to serve on the council a person must live within the LISD community, work for LISD, or have children that attend LISD. They must have a strong commitment to the health and well-being of children and a strong desire to attend and participate in the meetings.

Why do you want to become a member of the district's School Health Advisory Council *

The SHAC meets a minimum of four times a school year, typically on Wednesdays from 6pm to 7pm. Does your schedule allow you to make the majority of our meetings? *

The Texas Open Meetings Act requires SHAC meetings to be recorded and posted for public viewing. Do you agree to your audio and video image being recorded as a part of this requirement? *
