This form records how many Christian students participated in outreach during the school year.
Before you fill out this form, make sure you fill out the corresponding worksheet in your Campus Records Booklet.
Submit this form by 2 weeks before the college school year ends. This form only asks for the numbers of students who participated in each capacity and in total. It does not ask for the names of students.
