We are hosting a 3v3 Tournament Fundraiser on Sunday July 7th at MMU for children entering grades 4th-12th (and adults!)...

Inviting all children entering grades K-3rd to join the fun at a Halftime Exhibition from 12-1pm! There will be fun games, music, and special guests!

This will be a GREAT community event. Aside from this exhibition clinic, there will be: lots of exciting 3v3 games, a concession stand; food vendors; raffles; music; games & FUN!

In the event of a serious accident or illness concerning my child, I understand that the Mount Mansfield Youth Basketball League (MMYBL) will try to contact the adult listed using the information given on the registration form. If I cannot be reached, I authorize MMYBL to take whatever steps it deems necessary for the health, security, and comfort of my child. MMYBL hereby informs both the player and the parents that there are risks inherent in athletic participation. By registering and acknowledging the statements below the player and parents acknowledge this information and give their consent to participate. 
  1. I realize there is a risk of being injured in all sports. I understand the risks include severe injuries such as fractures, brain injuries, paralysis, or even death. We further understand that MMYBL disclaims any financial responsibility for the costs of medical treatment, hospitals, ambulances or paramedics, etc., arising from an injury to my child while participating in such competition or preparation, therefore.
  2. Whereas, the undersigned parent/guardian/adult having custody of the above mentioned minor child, being fully aware of the nature of the risk inherent hereby releases MMYBL, any of its elected or appointed officials, or any officers, coaches, sponsors referees, and CO-players from any and all liability for injuries, claims, demands, costs loss of services, expenses and/or damages which may be sustained by me or us or our minor child on account of his or her participation in said program or associated activities and events
BY ACCEPTING BELOW, WE ARE PROVIDING AN ELECTRONIC AGREEMENT THAT WILL HAVE THE SAME EFFECT AS AN ORIGINAL MANUAL PAPER SIGNATURE. THE ELECTRONIC ACCEPTANCE WILL BE EQUALLY AS BINDING AS AN ORIGINAL MANUAL PAPER SIGNATURE.
