FBS GIFT AID DECLARATION FORM
For UK taxpayers, under the HMRC Gift Aid scheme charities are able to claim back the tax you have paid on your donations (25p for every £1 you donate), making them worth 25% more to us, at no extra cost to you.

By completing this form ONCE you can authorise us to claim Gift Aid on ALL your donations made to Friends of Fulham Boys School AND/OR The Fulham Boys School Foundation, made now, in the future, or during the last four years.*

If you are NOT a UK tax payer please let us know too - at the bottom of the form - so that we can note your record and not trouble you with this again.

Once we have your declaration we will claim gift aid on all future donations, until you tell us otherwise.  It's really important that you tell us if you move, or if you are no longer a UK tax payer.

With grateful thanks,

Sal
