This survey is part of the Stay Safe Community Listening Campaign. The Stay Safe Community Listening Campaign provides safe space for survivors of gun violence, their families, and impacted community members to share their stories. This campaign is about listening deeply and gathering insights that will shape solutions, policies, and programs to prevent future harm.

The campaign is currently gathering input from survivors/victims of gun violence. Your answers to these questions are valuable to this effort. No identifying information will be shared publicly. Please do not include names or other identifying information. Information we collect will be summarized and shared as part of this campaign.
